Home & Personal Care
Homemaker/Personal Care, often called HPC.
An HPC supports a person with developmental disability to be more independent while meeting their daily living needs. Their sole purpose of the care taker is to advance the individual’s independence within his/her home and community and help the individual meet daily living needs.

Self Advocacy training to assist the expression of personal preferences, asserting rights, and making increasingly responsible choices.

Self-Direction making decisions and choices affecting the individual’s life,, Daily Living Skills, including training in and providing assistance with meal preparation, personal care, behavioral support plan, Community work force and education and self-administration of medication.

When provided with other components of homemaker/personal care, assist with personal finances.
Non-Medical Transportation
Non-Medical Transportation assists a person with transportation to their day services, employment services, or places of employment. We provide modified and non modified vehicles.
Non-Medical Transportation includes transportation to get to, from, between, or among:

A place of employment

A location where adult day support, career planning, group employment support, individual employment support, or vocational habilitation is provided to the individual

A volunteer activity

A post-secondary educational program

An internship or practicum

A drop-off or transfer location from which the individual is then transported to or from one of above listed places
In-Home Living + Respite Options

COE Supported Living program helps individuals with intellectual and developmental disabilities (I/DD) and behavioral health challenges, who live in their own homes or apartments, lead independent lives, and become active members of their communities.

Medicaid funds our Supported Living program through individual options (IO), Level 1 and Self waivers.
